*Campus Flu Shot Clinics for WISD Students and Staff Only. Clinics will last until the final student has been seen. If a parent wishes to be present at the time of the vaccination of their child, they need to be present at the beginning of the clinic.

Flu Shot consent forms will need to be filled out prior to receiving the flu vaccination. Forms can be found on the Health Services page on the Weatherford ISD website. www.weatherfordisd.com

Fee
 The Parker County Hospital District will file for most commercial insurance.
 $20 for those without insurance
 Age 6 months to 18 yrs old with insurance – insurance will be filed by Parker County Hospital District
 Age 6 months to 18 yrs without insurance or underinsured – no cost
WISD Employees
 TRS Aetna tiers ActiveCare 1-HD and ActiveCare 2 (or also listed as Aetna Choice POS ll) -- these tiers will cover PCHD administering the Flu vaccine at no cost to the employee.
 Anyone with Aetna Select (Open Access) Network or Aetna Whole Health (Baylor Scott & White) -- these plans will NOT cover PCHD administering the flu vaccine.

Weatherford ISD Volunteer Application

Mary Martin Elementary has a great volunteer program with a variety of opportunities for you to partner with our school. Our volunteers and mentors make a difference in the lives of students and provide essential support to our classroom teachers. We invite you to be a part of our volunteer program!

Weather you can give a little or a lot of time, your volunteer time is needed and appreciated by our students and staff. Our volunteer opportunities include tutoring and mentoring students, chaperoning field trips, serving as room parents, helping with field day, book fairs, PTA Carnival, helping with class projects, laminating, and helping in the campus work room. (All are very important to our campus)

You will find the form attached: please complete all of the forms and return to the campus office or volunteer coordinator, or you may email it to our campus. Please note the approval process may several days to complete. A list of approved volunteers is sent regularly to our Volunteer Coordinator who will contact you regarding the available volunteer opportunities.

Kindergarten Meet the Teacher/Orientation/School Supply Drop Off will be August 24th at 6:15 PM

Remember to stop by our main hallway and signup for PTA, School Bus Transportation and Kids Unite.
